What insurances are accepted?

Most commercial insurance plans are accepted.

What do I have to pay at appointments?

Payment for all services is due at the time of appointment including insurance deductibles, copays, and co-insurances.

Are cash discounts offered?

Yes.  Special pricing is available for non-insured patients for some services.

Are New Patients accepted?

Yes. New patients are accepted after screening to ensure required patient care can be provided based on patient needs.

Are lab services offered at the clinic?

Yes. On-site lab services are offered by our Clia Certified Moderate Complexity Lab and Certified Staff.

Lab hours are MondayThursday from 8:00am11:30am and 1:00pm – 4:30pm

Is New Patient Paperwork offered on-line?

Yes. You may access New Patient Paperwork under the Patient Forms Tab on our website.  You may print those documents, complete them and bring them with you to your appointment.

What am I required to bring to my appointments?

Current insurance card along with photo ID.

Am I required to keep my appointments?

Yes. Follow-up appointments are made in the interests of our patients continued good health and monitoring.  If appointments need to be rescheduled, we request 24 hours notice to reschedule or cancel.

Are my prescriptions sent electronically to pharmacies?

Yes.  Prescriptions are issued electronically to pharmacies in most cases.  Prescription requests are worked on a daily basis and patients are encouraged to check with their pharmacies for medication availability.

Will my phone calls be returned and my medical needs met?

Yes. Staff is required to return patient phone calls and complete medical tasks on a daily basis.  Ensuring our patient’s satisfaction is a priority, and we strive for perfection.  Any concerns or problems may be directed to the Practice Administrator, Laurie Rast at 903-669-0800.

Will I have online access to my Medical Records, and are they kept
securely?

Yes.  On-line Medical Record Access is available as a free service to all established patients who sign up with our Patient Portal.  Medical Records are maintained in a HIPAA compliant, secure manner.

How do I sign up for the Patient Portal, and why is this a valuable
service?

Existing patients may supply an email address so that we may send them an email invite to join our Patient Portal.  This service allows our patients to communicate with our healthcare team in a secure, convenient manner. In addition, patients may request appointments, pay bills, review medical records or print needed information.

May I email my provider?

No.  For the protection of our patients, we don’t utilize non-secure email services but do offer electronic patient communication as described above via our Patient Portal.
